The church members trust us to keep our children and youth safe. In order to keep that trust, we have a responsibility to evaluate the suitability of each volunteer who will be working with our children and/or youth. Please help us meet this responsibility by answering the questions below completely and honestly.

8: Have you ever been ordered to pay monetary damages in civil litigation or has money been paid by you or on your behalf to settle civil litigation against you for allegedly committing child abuse, sexual abuse, sexual harassment, sexual exploitation, or physical abuse?

9: While you were a juvenile, were you ever determined to be subject to supervision of any criminal justice authority due to sexual or violent behavior?

10: Since the age of 18, have you ever had sexual contact with person(s) you were seeing in your capacity as a professional (i.e. teacher/student, doctor/patient, clergy/parishioner, lawyer/client, counselor/counseled, supervisor/subordinate, etc.)?
